Failover attempts failed

Logged when failover finishes without a successful attempt. The attached error identifies the last failed server and cause, including timeouts. attempt_count gives the number of attempted servers. When the final attempt exceeded its deadline, timeout gives that limit and timeout_phase is attempt.

Check the named server and its Timeouts and Errors counters. Compare timeout with the backend action limit; investigate dependency latency and connectivity before increasing it. For other failures, follow the attached error.
