Closing RADIUS/1.1 backend connection after receiving a duplicate outstanding Token

RADIUS/1.1 peer reused a Token that still identifies an outstanding request.

Logged when a RADIUS/1.1 backend connection receives a request whose Token is already outstanding. Radiator closes the connection because the requests cannot be correlated safely.

Inspect the peer and packet fields. Correct a backend that reuses Tokens before completing the earlier request; repeated occurrences can also indicate duplicated or replayed traffic.
