Backend action timed out

Logged when a backend action reaches its timeout while queueing a request or waiting for the backend result. Radiator stops the action without trying another server.

The timeout_phase field is queue when the backend worker queue did not accept the request, worker_queue when the request was accepted but no worker claimed it, and reply when a worker claimed the request but did not finish before the deadline. The reply phase does not identify which operation inside the backend worker was waiting.

Check timeout_phase, server_name, backend counters, and backend latency. Investigate worker capacity for worker_queue or slow backend processing for reply before changing the backend action timeout.
