Closing Radius/1.1 connection after receiving a duplicate outstanding Token

Logged when a RADIUS/1.1 client request uses a Token that already identifies an outstanding request. Radiator closes the connection because the requests cannot be correlated safely.

Inspect the client and packet fields. Correct a client that reuses Tokens before completing the earlier request; repeated occurrences can also indicate duplicated or replayed traffic.
