Session authorization refresh failed

Logged when a cookie-authenticated request cannot refresh stale session authorization because the http_management_authentication @resolve pipeline failed, returned an error, timed out, or accepted the session without setting user.username. Requests for the same session fail without new backend lookups for five seconds, and the session stays active.

Check the error and session ID. Verify that the referenced identity backend is reachable and not throttling Radiator, and that the @resolve pipeline sets user.username.
