Diameter backend exchange failed

Logged when a Diameter backend has no usable runtime or peer, or its configured exchange fails. Accounting acknowledgement policies can still acknowledge the RADIUS request while this fault remains active. It is logged only when an alerts block enables backend incidents.

Check the attached error, configured peer group, peer connectivity and returned Diameter result. Restore a valid exchange for the affected operation.
