Additional Diameter connection monitor failed during shutdown

Logged for each connection monitor task after the first that panicked while its Diameter routing domain was shut down. The first failure is reported by the shutdown result; shutdown still joins every monitor and releases pending and duplicate-request state.

Report the attached error together with the first shutdown failure.
